Contractor Qualifications

Exceptional Contractors Are Critical to the Success of the EFS Program

To qualify, contractors must meet the following minimum requirements:

  • Complete a contractor application and meet the terms of the Contractor Participation Guidelines and Participation Agreement.
  • Undergo an annual evaluation by EFS after initial approval.
  • Be in business for at least one year.
  • Possess all necessary licenses and permits.
  • Carry $1 million in general liability insurance and worker's compensation as required by law.
  • Maintain a "Satisfactory" rating or no rating with the Better Business Bureau.

Application Process

Please submit the following documents via email to or by fax to 608.249.5788. You may also send them via regular mail to 431 Charmany Drive, Madison, WI 53719.

  • Contractor Application
  • IRS W-9 Form
  • Contractor Participation Agreement
  • Certificate of Insurance
  • Contractor Portal Use Agreement (if desired)
  • ACH Authorization (if desired)

Once all contractor application forms have been reviewed by EFS, you will be notified of our decision via email. Turnaround is typically 1-2 business days.
